Please be on time to optimize your Physical Therapy session. Please understand that there are other patients that need treatment

Please call if you will be late. We will do our best to fit you in - Please show up 15 minutes prior to the appointment to fill out the appropriate paperwork

Allow 1 hour for the examination

Bring comfortable clothing so the area can be fully exposed for examination and treatment. We provide changing facilities

We are not responsible for lost items

Bring all appropriate paper work if required and or needed

Insurance card

Prescription for Physical Therapy (if you have one, but not necessary) - List of medications

Bring prescribed medications that are needed for exercise

All minors under 18 must be present with parent/guardian for at least the first session

Upon arrival, all patients must fill out a Health Insurance Portability and Accountability Act (HIPAA) form and a health questionairre form before examination

All visitors must wait in the waiting room

Please give 24 hours notice when possible for cancellations. If it is off hours, please leave a message on the answering machine

If a patient no shows more than three times, we hold the right to deny treatment and your doctor will be informed of your
non-compliance

We hold the right to discontinue physical therapy should the patient be in non-compliance and/or has reached maximum potential for skilled physical therapy services

-Please notify us at least one week prior to returning to your physician - No cell phones in the treatment area
