 Policy and Procedures

- Please call to make an appointment (201) 393-9038. We will do our best to see you within 24 hours

- Please be on time to optimize your Physical Therapy session. Please understand that there are other patients that need treatment

- Please call if you will be late. We will do our best to fit you in - Please show up 15 minutes prior to the appointment to fill out the appropriate paperwork

- Allow 1 hour for the examination

- Bring comfortable clothing so the area can be fully exposed for examination and treatment. We provide changing facilities

- We are not responsible for lost items

- Bring all appropriate paper work if required and or needed

- Insurance card

- Prescription for Physical Therapy (if you have one, but not

necessary) - List of medications

- Bring prescribed medications that are needed for exercise

- All minors under 18 must be present with parent/guardian for at least the first session

- Upon arrival, all patients must fill out a Health Insurance Portability and Accountability Act (HIPAA) form and a health questionairre form before examination

- All visitors must wait in the waiting room

- Please give 24 hours notice when possible for cancellations. If it is off hours, please leave a message on the answering machine

- If a patient no shows more than three times, we hold the right to deny treatment and your doctor will be informed of your non-compliance

- We hold the right to discontinue physical therapy should the patient be in non-compliance and/or has reached maximum potential for skilled physical therapy services

- Please notify us at least one week prior to returning to your physician - No cell phones in the treatment area

- Respect your fellow patients

- Please do not hesitate to contact our office at any time if you have any questions or concerns
