All clients/ legal guardians/physicians must fill out and
sign profile forms, consent forms, medical releases (via Medical Doctor if
appropriate) prior to participation.
All clients are to bring any medication/home remedies that may
be needed in case of an emergency i.e. insulin, sub- lingual nitrate, candy and
or juice. These medications/supplements will be self-administered. It is not the
responsibility of IFSI to administer medication; however, we will call 911 if
necessary.
All clients must inform the trainer if anything is
bothering you prior to, during or after each session
All clients must be on time and ready to perform
All clients must wear loose fitting clothing that is appropriate
in cover, laced sneakers, socks, and no jewelry
All clients must not eat heavy and full meals prior to training
All clients should hydrate before each session and bring water
to continue hydration throughout the session
Cancellations are understood however, if
IFSI is not informed 24 hours prior to the scheduled session it is
considered a session and will be deducted from your package
Full payment is expected prior to the start of any programs
All clients must respect other athletes and
IFSI’s organization. Vulgar/inappropriate language
and gestures are not permitted and is grounds for discharge from the program
All information dispensed either in handouts or programs being
performed from IFSI are considered copyrighted and must have the written consent
from IFSI to note take, copy or dispense
It should be remembered that each athlete is individual,
possessing individual abilities. Respect is expected. All clients will act as
team members, respecting differences and praising hard work efforts
The training sessions are off limits for any
observation/interaction with clients unless there is a prior arrangement with
IFSI

It is our performance training policy that all clients
must have a comprehensive Pre-Medical and Performance Athletic Screening prior
to entering our programs. This is called the Medical and Performance Profile
(MAPP). The MAPP will include but not limited to posture, gait analysis, special
movement tests, cardiovascular endurance, measurements of muscular strength,
flexibility, and if indicated, a body fat analysis. (Please note portions may
vary depending on history and /or purpose of training). Please note the data
collected may be used in confidence for the purpose of research, outcome studies
and for professional intervention needed for the best health and well being of a
client.

IFSI holds the right to discontinue services at any time if

20% of the remaining prorated package balance will be withheld
if policy and procedure is trespassed and/or the athlete is asked to discontinue
service due to improper behavior
